At Care and Case Management Services LTD (CCMS), we offer very distinct and specialist support services for adults and children recovering from an acquired or traumatic brain injury, spinal injury, catastrophic injuries, and associated disabilities.

CCMS was founded in 2007 and was based in Stokesley, North Yorkshire. We have since moved to Darlington. Our team consists of 9 case managers from varying clinical backgrounds and 8 operational staff members.
We pride ourselves in offering a tailored, client focused and professional service for clients of all ages, following catastrophic injury. Our highly experienced case managers and support staff come from varying backgrounds which benefits our clients hugely as we pull on all our resources to work as part of a team and to ensure the best outcome for our clients and their families.
It is paramount for us to have a good relationship with not only our clients and their family members but also support workers and therapists too, as every person involved plays an important role in enabling our clients to move forwards and to achieve their goals in a well-supported environment.
We support clients across the whole of the Northeast of England and Yorkshire.

We have been rated as ‘Outstanding’ by the Care Quality Commission. However, this certainly doesn’t mean that we have sat back and put our feet up!
Our aim is to continuously exceed the expectations of not only our clients but the care professionals we work alongside. We are committed to an open, honest and transparent approach in all areas. To achieve this, we make sure there is regular client contact with documents meetings and reviews of relevant documentation, clinical governance reviews and training and development of all staff that work at CCMS.
