Job Title: CASE MANAGER

The Role

We have two Case Manager opportunities for qualified professionals such as: Social Worker, Nurse, Occupational Therapist, Physiotherapist, Speech and Language Therapist, Counsellor, Psychologist.

CCMS is an award-winning case management service with a CQC Outstanding Rating, we are regulated under Personal Care and TDDI, specialising in the rehabilitation of adults & children following brain injury, spinal injury and associated catastrophic injuries.

As Case Managers, we continuously work with our clients and their families to assess rehabilitation needs, report to court, commission services and lead multi-disciplinary teams, providing rehabilitation and support in the community to assist clients to reach their optimum potential.

We are seeking dynamic, passionate individuals with the following:

Professionally qualified in roles such as Social Worker, Nurse, Occupational Therapist, Physiotherapist, Speech and Language Therapist, Counsellor or Psychologist. Experience as a Case Manager would be advantageous but is certainly not essential as full training is provided.

 A structured and logical thinking style, operating both at a detailed level and at a directional level
Excellent report writing skills
Highly effective verbal and written communication skills
Ability to build therapeutic relationships quickly and have strong interpersonal skills with clients and professionals
Ability to work autonomously and also as part of a team
Ability to demonstrate excellent time management skills and also co-ordinate others when working to deadlines
Ability to apply new learning to work situation
Ability to transition from your current role into the role of Case Manager
Must be IT literate and have the ability to learn new systems quickly
Strong problem-solving skills and the ability to manage difficult and at times, volatile situations

Salary: Very competitive dependent on knowledge and experience.

      The Role

      Support worker required for a gentleman in his 60’s living independently in the Penrith area who has an Acquired Brain Injury.  The successful applicant will be responsible for supporting the client and promoting independence with daily living tasks including household chores, personal care, shopping and regularly going out in the local community.

      Applicants are sought with the following qualities: good communication skills, patience, problem solving ability, and a sense of humour.

      The successful applicant will join an established support team and will receive multi-disciplinary guidance and supervision.

      Full training provided

      Full UK driving license essential.

      Experience preferred but not essential

       

      Job Title: 2 x FEMALE CARE ASSISTANT

      Hours: 20hrs per week

      The Role

      We are currently recruiting for a Care Assistant to join a team for a lovely lady in the York area to maximise her independence following a road traffic accident in 2015.

      She has a spinal cord injury resulting in Tetraplegia and at present has significant mobility problems.  She is living in Stamford Bridge, York. The client enjoys cooking, baking and participating in a well established exercise programme.

      Full personal care required. 

      Due to the length of the shifts we believe this opportunity is commutable from – Leeds, York, Thirsk, Helmsley and Hambleton District 

      Applicants require an empathetic & dynamic approach, with the ability to promote and encourage therapies.  You will be required to engage the client in home & community activities. 

      Experience preferred with moving and handling tasks. Driver/full licence essential. 

      Flexible approach necessary with regards to working shift patterns.

      Successful applicants will receive support, training & supervision.

       

      Job Title: FEMALE CARE ASSIATANTS

      The Role

      We are recruiting for Female Care Assistants to support a 14 year old girl with Cerebral Palsy.  The client lives in the Sunderland area within her family home.

      The role will involve enabling the client to engage in rehabilitation, social and recreational activities, and empowering her to be as independent as possible in the home and in the community. The client enjoys swimming, live music and trampolining.

      Flexible support required after school, weekends and during school holidays.  Shifts will vary depending upon the client’s needs and activities.

      Care experience with young people is desirable and applicants must be enthusiastic, energetic and have a good sense of humour.

      The client requires full moving and handling and all aspects of personal care.    

      The successful applicant will receive full training for this role, input from Therapists and regular supervision by Case Manager.

      Driver with full UK License essential.

      The Role

      We are looking for a Health Care Assistant(s) for a 10 year old young man living in in his

      family home and in school in Honley.

      The ideal candidate will have some experience in Complex Care for children with cerebral palsy, enteral feeding and epilepsy, but full training will be provided.  

      Shifts required will be both weekdays and weekends. Including supporting in the school environment.

      Some daytime shifts during school holidays will be double up with other carers.

      Candidates must be flexible and willing to travel as they may be required to accompany the young boy and his family to their caravan in Blackpool.

      His ideal carer would be calm and friendly, (he loves being talked to). He loves stories, songs, messy play and bath times.

      Successful applicant will join a team of professionals & receive support, training & supervision.

       Car driver and own vehicle essential.

      Job Title: Personal Care Assistant

      The Role

      Personal assistant required to provide support for a 19 year old male with cerebral palsy, affecting all four limbs living in his family home in the North Sheffield area.

      The care plan aims to maximise the client’s quality of life and independence, by facilitating engagement in activities of daily living, hobbies and accessing the community.

      We require Monday afternoons plus sleep-in shift per week plus additional days in school holidays to cover MAT leave, with the potential for more shifts and a permanent position.

       Moving and Handling and personal cares required.

       Full Driving licence preferred for use of client’s own vehicle.

       Applicants should be empathetic, committed, reliable, motivated, creative, patient, emotionally resilient and persistent, and be able to work alone and as part of a team.  

       Successful applicants will receive full support, training and

      supervision from the Case Manager.
