The Role

We have two Case Manager opportunities for qualified professionals such as: Social Worker, Nurse, Occupational Therapist, Physiotherapist, Speech and Language Therapist, Counsellor, Psychologist.

CCMS is an award-winning case management service with a CQC Outstanding Rating, we are regulated under Personal Care and TDDI, specialising in the rehabilitation of adults & children following brain injury, spinal injury and associated catastrophic injuries.

As Case Managers, we continuously work with our clients and their families to assess rehabilitation needs, report to court, commission services and lead multi-disciplinary teams, providing rehabilitation and support in the community to assist clients to reach their optimum potential.

We are seeking dynamic, passionate individuals with the following:

Professionally qualified in roles such as Social Worker, Nurse, Occupational Therapist, Physiotherapist, Speech and Language Therapist, Counsellor or Psychologist. Experience as a Case Manager would be advantageous but is certainly not essential as full training is provided.

 A structured and logical thinking style, operating both at a detailed level and at a directional level
Excellent report writing skills
Highly effective verbal and written communication skills
Ability to build therapeutic relationships quickly and have strong interpersonal skills with clients and professionals
Ability to work autonomously and also as part of a team
Ability to demonstrate excellent time management skills and also co-ordinate others when working to deadlines
Ability to apply new learning to work situation
Ability to transition from your current role into the role of Case Manager
Must be IT literate and have the ability to learn new systems quickly
Strong problem-solving skills and the ability to manage difficult and at times, volatile situations

Job Title: Personal Care Assistant

The Role

Personal assistant required to provide support for a 19 year old male with cerebral palsy, affecting all four limbs living in his family home in the North Sheffield area.

 The care plan aims to maximise the client’s quality of life and independence, by facilitating engagement in activities of daily living, hobbies and accessing the community.

 We require 2 x afternoons plus overnight sleep-in shifts per week term-time 3.30pm – 08.30am. Shifts may vary during school holidays and are planned/agreed in advance.

 Rate of pay will be reviewed following successful completion of 6 month probationary period.

 Moving and Handling and personal cares required.

 Full Driving licence preferred for use of client’s own vehicle.

The Role

We are recruiting for Care Assistants to support a 5 year old boy with Cerebral Palsy.  The client lives in the Newcastle area within his family home. The role will involve enabling the client to engage in rehabilitation. He requires assistance with all activities of daily living

Care experience with young people is desirable The ideal candidate will have some experience in Complex Care for children with cerebral palsy, visually impairment, cystic fibrosis and gastrostomy feeding, but full training will be provided.  

 The client requires full moving and handling and all aspects of personal care 24/7.     

The successful applicant will receive full training for this role, input from Therapists and regular supervision by Case Manager.

Shifts required will be both weekdays and weekends. Waking night shifts will also be included. The hours are negotiable.

The Role

Support Worker required to provide support to a 26-year-old male with an acquired brain injury living in his family home in the Sedgefield area. The client requires support for 2 days per week to access the community and recreational activities. 

The client requires support to engage in recreational activities in the community and attending Headway sessions.

The client would like his Support Worker to preferably be of similar age or to have similar interests such as watching football, superheroes, comic-con, going to the cinema, bowling, and attending the gym. The client also enjoys trips away to London and therefore flexibility is also required to support the client on short breaks.

The role will involve enabling the client to engage in rehabilitation activities and empowering him to help increase his motivation, self-esteem, and confidence to be as independent as possible.

The successful applicant should be enthusiastic, have excellent communications skills and have the skills to motivate the client to engage in a range of activities outside of the home.

Driver essential with use of own vehicle to take the client out in the community.

Previous experience desired but not essential as full training provided.

The Role

We are currently recruiting for a Care Assistant to join a team for a lovely lady in the York area to maximise her independence following a road traffic accident in 2015.

She has a spinal cord injury resulting in Tetraplegia and at present has significant mobility problems.  She is living in Stamford Bridge, York. The client enjoys cooking, baking and participating in a well established exercise programme.

Full personal care required. 

Due to the length of the shifts we believe this opportunity is commutable from – Leeds, York, Thirsk, Helmsley and Hambleton District 

Applicants require an empathetic & dynamic approach, with the ability to promote and encourage therapies.  You will be required to engage the client in home & community activities. 

Experience preferred with moving and handling tasks. Driver/full licence essential.

The Role

We are currently recruiting for a Care Assistant to join a team for a young lady in the York area.

The client contracted Meningitis in 2010 which resulted in an incomplete Spinal Cord Injury  causing quadriplegia and a moderate Brain Injury.

She is living independently in Melbourne, York and requires support with Rehabilitation and Therapies.

Full personal care required. 

Moving and handling experience preferable but not essential.

Applicants require an empathetic & dynamic approach, with the ability to promote and encourage therapies.  You will be required to engage the client in home & community activities.

The Role

We are recruiting for Female Care Assistants to support a 14 year old girl with Cerebral Palsy.  The client lives in the Sunderland area within her family home.

The role will involve enabling the client to engage in rehabilitation, social and recreational activities, and empowering her to be as independent as possible in the home and in the community. The client enjoys swimming, live music and trampolining.

Flexible support required after school, weekends and during school holidays.  Shifts will vary depending upon the client’s needs and activities.

Care experience with young people is desirable and applicants must be enthusiastic, energetic and have a good sense of humour.

The client requires full moving and handling and all aspects of personal care.    

The successful applicant will receive full training for this role, input from Therapists and regular supervision by Case Manager.

The Role

We are looking for a Health Care Assistant(s) for a 10 year old young man living in in his family home and in school in Honley.

The ideal candidate will have some experience in Complex Care for children with cerebral palsy, enteral feeding and epilepsy, but full training will be provided.  

Shifts required will be both weekdays and weekends. Including supporting in the school environment.

Some daytime shifts during school holidays will be double up with other carers.

Candidates must be flexible and willing to travel as they may be required to accompany the young boy and his family to their caravan in Blackpool.

His ideal carer would be calm and friendly, (he loves being talked to). He loves stories, songs, messy play and bath times.
