The Role

We have two Case Manager opportunities for qualified professionals such as: Social Worker, Nurse, Occupational Therapist, Physiotherapist, Speech and Language Therapist, Counsellor, Psychologist.

CCMS is an award-winning case management service with a CQC Outstanding Rating, we are regulated under Personal Care and TDDI, specialising in the rehabilitation of adults & children following brain injury, spinal injury and associated catastrophic injuries.

As Case Managers, we continuously work with our clients and their families to assess rehabilitation needs, report to court, commission services and lead multi-disciplinary teams, providing rehabilitation and support in the community to assist clients to reach their optimum potential.

We are seeking dynamic, passionate individuals with the following:

Professionally qualified in roles such as Social Worker, Nurse, Occupational Therapist, Physiotherapist, Speech and Language Therapist, Counsellor or Psychologist. Experience as a Case Manager would be advantageous but is certainly not essential as full training is provided.

 A structured and logical thinking style, operating both at a detailed level and at a directional level
Excellent report writing skills
Highly effective verbal and written communication skills
Ability to build therapeutic relationships quickly and have strong interpersonal skills with clients and professionals
Ability to work autonomously and also as part of a team
Ability to demonstrate excellent time management skills and also co-ordinate others when working to deadlines
Ability to apply new learning to work situation
Ability to transition from your current role into the role of Case Manager
Must be IT literate and have the ability to learn new systems quickly
Strong problem-solving skills and the ability to manage difficult and at times, volatile situations

The Role

Personal assistant required to provide support for a 20 year old male with cerebral palsy, affecting all four limbs living in his family home in the North Sheffield area.

The care plan aims to maximise the client’s quality of life and independence, by facilitating engagement in activities of daily living, hobbies and accessing the community.

We require afternoon support plus overnight sleep-in shifts per week term-time 3.30pm – 08.30am. Shifts may vary during school holidays and are planned/agreed in advance.

The Role

An exciting opportunity has arisen to join a 24/7 care team supporting an 14 year old boy in the family home, school and community.

Our client has Autism and Cerebral Palsy who requires 1:1 support to provide care, guidance, encouragement, and supervision to maintain safety.

Meeting his health and social care needs at home, school and in the community including swimming, family holidays etc

He is a power wheelchair user and requires assistance with personal care.

The successful applicant will provide support to engage in activities, attend school complete homework and access the community either with or without the family.

During school holidays support will be required in the home and community

 flexibility will be required.

The client requires full moving and handling and all aspects of personal care.

The Role

We are currently recruiting for a Care Assistant to join a team for a lovely lady in the York area to maximise her independence following a road traffic accident in 2015.

She has a spinal cord injury resulting in Tetraplegia and at present has significant mobility problems.  She is living in Stamford Bridge, York. The client enjoys cooking, baking and participating in a well-established exercise programme.

We believe this opportunity is commutable from – Leeds, York, Thirsk, Helmsley, Pocklington and Market Weighton.

The Role

The Role

A support worker is required to join an existing 24/7 team for a gentleman in his 70’s with an acquired brain injury in the Hexham area. The client has cognitive problems and some mild challenging behaviours following a road traffic accident, he requires 1;1 support and supervision at all times in his own home and the local community.

The care plan aims to maximise the client’s quality of life, by facilitating engagement in activities of daily living and providing reassurance and encouragement.

The role requires assistance with personal care tasks.

The role includes 24 hour shifts including sleeping nights which are in addition to the contracted hours. 21 hours contracted hours per week equates to 3×24 hour shifts per fortnight (including sleepovers) and 35 hours per week equates to 5×24 hour shifts per fortnight. A rolling rota system is in place, therefore shift requirements are planned well in advance but can be swapped where required.

Applicants should be reliable, patient and have excellent communication and observational skills

Previous experience working with clients with brain injury is preferable.  

Driver with full UK driving license essential as the client has his own car for the team to take him out in.

The Role

We are currently recruiting for a Female Bank Support Worker to join a lovely team for a client in the Durham area.

The client is a lady in her 50s who had a brain injury a number of years ago. She is independently mobile and can communicate fully. She requires prompting and supervising with her personal care, making meals and day to day activities. The lady loves music, the theatre, jigsaws, puzzles and movies.  

The client has poor short term memory and can become frustrated. Any applicant needs to have patience and empathy. A background in ABI/support work is desirable but not essential.

Driver/full license essential. 

As this is a Bank position, applicants must have a flexible approach and be able to cover a mix of weekdays and weekends. Shifts will be offered when covering holidays, sickness, training etc and for this reason sometimes shifts will be offered at short notice.

The Role

 Flexible support required 24/7. The package will include 24/7 care during holidays and weekends

 We are seeking staff with excellent communication skills and a desire to provide individualized support.  Previous care experience with children is preferred. Staff must be able to participate in a range of activities with the client such as recreational activities.

The Role

We are looking for a Health Care Assistant(s) for a 11 year old young man living in in his family home and in school in Honley.

The ideal candidate will have some experience in Complex Care for children with cerebral palsy, enteral feeding and epilepsy, but full training will be provided.  

Shifts required will be both weekdays and weekends. Including waking overnight shifts and supporting in the school environment. 

Some daytime shifts during school holidays will be double up with other carers.

Candidates must be flexible and willing to travel as they may be required to accompany the young boy and his family to their caravan in Blackpool.

His ideal carer would be calm and friendly, (he loves being talked to). He loves stories, songs, messy play and bath times.
