Rachel Cromack

Case Manager

Rachel has joined CCMS with over 22 years’ experience as a Registered Children’s Nurse and two years’ experience as a Case Manager. She began her career on a busy children’s medical ward within the NHS, developing skills in assessing, planning, implementing and evaluating nursing care. After two years, she moved on to Paediatric Intensive Care Unit (PICU) where she quickly progressed to Sister, leading the team in looking after a wide variety of critically ill and injured children aged 0 – 20 years. She was an Advanced Paediatric Life Support provider, attending critical events and arrest situations within A&E, the wards and PICU. She developed a strong interest in brain injury, spinal injuries and long-term ventilation throughout her 18 years in PICU. In her last year of working in the NHS, she worked on Adult ITU to help in the Covid-19 crisis. This helped her to move her career into the community as a Complex Nurse Specialist, looking after both paediatric and adult clients with complex health needs, in their own homes. Within this role she used her skills to teach and assess carers in delivering safe and effective care to the clients and their families. Within this role she was the lead nurse for clients with spinal injuries.

Rachel became a Case Manager in 2021 and managed a caseload of clients, both pre and post litigation. She gained experience in completing INA’s, reports, case management plans, sourced and worked with a wide MDT, recruited and managed teams of support workers and developed good therapeutic relationships with her client’s and their families. Rachel is a member of the British Association of Brain Injury Case Managers (BABICM) and registered with the Nursing and Midwifery Council (NMC) pin: 97I2965E.
